What is the Fotona 4D Laser Facelift?
Fotona 4D is a series of non-invasive laser treatments performed on both the exterior of the face and the interior of the oral cavity. That dual approach is what sets it apart: by treating the tissue behind your nasolabial folds and perioral area from the mucosal side, the laser tightens structures that surface-only devices simply cannot reach. The treatment combines two wavelengths — Er:YAG at 2940 nm and Nd:YAG at 1064 nm — across four modes:
- Step 1 — SmoothLiftin®: Non-ablative Er:YAG delivered intraorally to stimulate collagen behind the cheeks, nasolabial folds and lips.
- Step 2 — FRAC3®: Short-pulse, high-peak-power Nd:YAG that creates a self-organizing three-dimensional fractional pattern, concentrating its effect on imperfections while sparing healthy tissue.
- Step 3 — PIANO®: Seconds-long Nd:YAG pulses that deliver deep bulk heating and a dermal heat-shock effect while leaving the epidermis untouched.
- Step 4 — SupErficial®: A light Er:YAG peel that finishes the treatment with immediate brightness and refined texture.
4D = 4 modes = 4 steps. Each one addresses a different depth, and together they produce a measurable, uniform tightening effect across the whole face.
How Does Fotona 4D Work?
Every step targets a different layer. The intraoral SmoothLiftin step uses Fotona’s patented SMOOTH® mode — a rapid burst of non-ablative micro-pulses delivered over roughly 250 milliseconds. Rather than removing tissue, this sequence heats the soft tissue to a depth of 200–500 microns, producing a controlled thermal stimulus that triggers collagen remodeling and neocollagenesis. Because it works through the inside of the cheek, it lifts the nasolabial region from behind. FRAC3 then addresses the outer surface. Its very short pulses (0.1–1.6 ms) and high peak power create microscopic islands of thermal effect that form preferentially at the sites of existing skin imperfections — pigmented spots, vessels, textural irregularities — which is why it is described as self-induced fractional treatment. Passes are laid down in a crosshatch across the full face, with additional passes along the natural vector lines of the lift. PIANO goes deepest. Its pulses last seconds rather than milliseconds, at low power, so heat accumulates in the dermis and subdermis while the epidermis stays cool and intact. This is the bulk-tightening and volumizing engine of the treatment. SupErficial closes the session with a very light ablative Er:YAG pass that polishes the surface, evens tone and leaves the skin looking immediately fresher.

What to Expect During a Fotona 4D Treatment
- Consultation: Dr. Kaveh evaluates your skin laxity, volume loss and texture, reviews your medical history, and confirms you’re a candidate.
- Preparation: Your face is cleansed and makeup removed. Protective eyewear is worn throughout — and for the intraoral step, the handpiece is fitted with a dedicated intraoral adapter.
- Treatment: The four steps are performed in sequence. Most patients need no anesthesia for the intraoral step. You’ll feel warmth and a mild prickling sensation, particularly during the FRAC3 pass; contact cooling is available and used as needed.
- Duration: A full four-step session typically runs 60 to 90 minutes.
You may look slightly flushed for a few hours afterward. There is no true downtime — most patients go straight back to work, errands or dinner plans.

- Avoid sun exposure and tanning beds for at least two weeks before treatment.
